Every record is a projection of a public-domain federal file (2026-08-10 release).
Nothing here is user-submitted, editorial, or ranked: listings are alphabetical,
and where a provider carries a CMS star rating it is reproduced with attribution
rather than aggregated into a score of our own. CMS itself notes that issuance of
an NPI does not verify that a provider is licensed or credentialed, and this
directory repeats that caveat rather than implying verification it cannot do.

## What each provider page carries
Practice address (USPS-normalised, with a flag when the ZIP contradicts the city),
phone, NPI, enumeration date, primary and secondary NUCC taxonomies, state licence
numbers, sole-proprietor and Medicare assignment status, group practice, medical
school and graduation year where CMS publishes them, and CMS Care Compare star
ratings for hospitals, nursing homes, dialysis facilities, hospices, inpatient
rehabilitation facilities and long-term care hospitals.
