NPI Guide does not require an account and does not ask for personal information to browse. We keep short-lived server logs (IP address, page requested, user agent) for security and capacity planning.

Provider records

Provider information shown here is published by CMS as FOIA-disclosable data. It concerns providers in their professional capacity — business addresses and business phone numbers — not private residential details.

Removal requests

We cannot alter the underlying federal record. Providers who want their details changed should update them in NPPES; the correction propagates here automatically. If a listing exposes something that is genuinely a private home address, write to contact@npiguide.com and we will suppress it pending the NPPES correction.
